Longitudinal Clinical Summary View


The most quoted goal of health information exchanges is to facilitate the seamless and effective handoff of clinical information between healthcare providers during care transitions. In the example illustrated here, a patient visits her primary care provider for a routine visit. The patient is referred by the PCP to a specialist for a consultation. The specialist diagnoses a condition that requires surgery. The patient is subsequently admitted to the hospital for surgery, attended by the specialist. The patient then returns to follow on care by the PCP. In each of the care transitions mentioned, the HIE provides access to the real-time and up-to-date longitudinal care record. The HealthUnity HIE solution supports this scenario out of the box. This service can be accessed using our HIE web client or natively from within any EHR application using user interface integration or interface level integration.

Record Location Service


The record location service is a variation of the longitudinal clinical summary service. The difference is that in the case of clinical summary, a pre-determined report is generated while RLS represents a filtered set of data presented in the consumer’s favored format. Secure Messaging


Traditional email does not encrypt messages. Email typically moves through untrusted systems. The secure messaging application provides a private communication mechanism for HIE participants. This service allows HIE nodes to directly exchange encrypted messages including attachments. Available features include composing, reading, archiving, forwarding and attachments of documents and clinical messages.

Provider-to-Provider Referral


It is common for healthcare providers to refer patients to other providers. The most common scenario of referral occurs when a patient is referred to a specialist by a primary care physician. Today, when such a referral happens, it is difficult to forward clinical information from the PCP to the specialist. Furthermore, when the patient returns from the specialist visit, it is difficult to get the results back. The HealthUnity HIE solution provides built in capability to enable electronic referral of patients from any provider to any other provider. Lab ordering and resulting is one of the most common transaction that occurs when a patient visits his doctor. The HealthUnity HIE supports the routing of orders from the provider to the lab and the result from the lab back to the provider. Supported features include code mapping, intelligent routing and importing into EHR systems.

This information is later used for generating reports that help in such scenarios like determining compliance with guidelines, school admission etc. The HealthUnity HIE solution supports applications that generate immunization information, such as Electronic Health Record systems, to access current records as well as update and add to existing records in the registry. This is performed using standards based transaction messages supported by the immunization registry.

Public health departments across the nation have strict reporting requirements that providers have to comply with. When a reportable adverse lab result is received, it is the duty of the provider to inform public health officials about the relevant information. In many cases, this includes the full demographics of the patient as well as the reportable lab result. The HealthUnity HIE solution allows for the easy reporting of suspected lab result either directly by the lab itself or by the ordering physician. Public health officials can then review this information using a web based user interface which is part of the HIE or import this information into their existing systems.
